Register for a Class

Please fill out the form below to register for one of our courses. Complete one form for each student. When you complete the fields below, click "Register" to send your request to EMS. Payment can be made by credit card and PayPal below, or by company check mailed to the EMS. We accept cash or company check at the training location. Please keep in mind that your seat WILL NOT be guaranteed until full payment is received.

Classes will be held at Cucina Biagio, 7319 W. Lawrence Ave, Harwood Heights, IL (1 block west of Harlem Ave.) Continental breakfast and lunch are included.

OSHA 10-Hour Course $435 per attendee
(fee includes continental breakfast and lunch)

Policy:

Payment in full must be received before a Certificate of Completion and OSHA card can be issued.. If a course is cancelled by EMS due to low enrollment, full payment will be refunded. Fees may be transferred to another student with a minimum of one day's notice. There is a $50 non-refundable processing fee for cancellation
